MyChart Terms of Use

DO NOT USE THIS SITE FOR EMERGENCY MEDICAL NEEDS. IF YOU EXPERIENCE A MEDICAL EMERGENCY, CALL 911 IMMEDIATELY.

You have been granted access to the MyChart patient portal (the "Portal") for SCL Health, also known as Intermountain Health, and its affiliates' ("SCL Health"). For affiliated patients, SCL Health also grants Portal access to health care providers ("Customers") enrolled in SCL Health’s Community Connect Program.

Please read the following information carefully. This notice (the "Terms of Use") includes patient consent and acknowledgement. You must indicate your agreement to these Terms of Use by clicking the "agree" checkbox at the end of this Terms of Use. If you do not agree, you will not be permitted to use this Portal.

SCL Health reserves the right to modify, amend or update these Terms of Use at any time without notice. Any updates to these Terms of Use will be posted on this Portal. You acknowledge and agree that it is your responsibility to review these Terms of Use so that you are aware of any changes. Your use of this Portal after such posting constitutes your acceptance of the revised Terms of Use.

Please print and keep a copy of the Terms of Use for your records.

SCL Health is pleased to offer you patient information access via a Web-based version of our computer systems Portal, called MyChart. This includes information related to a patient's current medications and appointments and allows patients to submit questions to SCL Health providers online.

SCL Health offers patients the option of non-urgent care telemedicine visits ("video visits"). Telemedicine involves a secure, online exchange of medical information between you and an SCL Health employed physician, physician assistant, or advanced practice nurse, right from the comfort of your own home or office. To be eligible for video visits, you must indicate your agreement to the additional Terms of Use for Telemedicine Non-Urgent Care Visit (video visits) below, which include patient consent and acknowledgement of financial obligations. Patients will be expected to pay for video visits at the time of the service, unless these services are covered by your insurance, employer, or other source.

The information provided on MyChart does not substitute for the advice of your personal physician or other qualified healthcare professionals. Always seek the advice of your physician or other healthcare professional with any questions you have regarding medical symptoms or conditions. Never disregard professional medical advice or delay in seeking it because of something you have read on MyChart.

For all urgent medical matters, contact your individual physician's office, go to an emergency room, and/or call 911. Never send any messages requiring urgent attention using MyChart. Delays can occur based upon, but not limited to, volume and availability of staff, the complexity of the patient's condition, or technology occurrences outside of SCL Health.

Customer Service Information

To request an account, you may sign up online, or in person at your Care Site or Physician Office.

To deactivate your MyChart account, you may notify your Care Site Health Information Department, notify your Physician Office or deactivate your MyChart account in the Security Settings within MyChart.

If you need information or technical assistance regarding MyChart or if you feel your login ID or password has been compromised, please contact the Customer Service Line at 1-855-274-2517 and select the MyChart option.

If you feel that information found in your MyChart record is inaccurate or incomplete, feel free to address your questions first to the physician's office where you receive care. If your questions or concerns persist after doing so, or if you think that information in your health record should be changed, please contact the individual noted on the Notice of Privacy Practices provided to you at your Care Site or Physician Office.

We reserve the right to terminate a user's access to MyChart at any time, for any reason. A user's access to MyChart will be terminated if the user has misused or abused the system, violated any terms of use, or used the system for any purposes other than intended uses.

Electronic Messaging

SCL Health hospitals' and clinics' goal is to always provide a timely response to electronic inquiries. In some cases, our staff may not be immediately available, so a patient should allow at least three (3) business days for the message to be addressed. MyChart does not allow you to message any healthcare providers that you have seen in a hospital setting. Clinic messaging is available to communicate with a provider in a clinic with whom you have an existing relationship.

Messages sent via MyChart may be viewed by other healthcare professionals who work with your physician; therefore, use of MyChart may not be appropriate for sensitive information you wish to discuss only with your provider.

Furthermore, with respect to any electronic communications sent by the patient, we are only able to respond to such communications based on the information provided by the patient. If there is insufficient information provided, we may be unable to provide accurate and reliable services.

Minor Eligibility

Parents or guardians of minor children may request access to the minor child's account by following the terms outlined in the "Proxy Access" sections of this Terms of Use statement. Minor children between the ages 12 and 17 (inclusive) will not be able to participate in the full spectrum of MyChart services. Under state and federal law, there are certain types of medical information that the parent or guardian of a minor patient may not view without consent of the minor patient. Given these requirements, restricted MyChart access will be offered to parents and guardians of patients age 12 or older. Thus, when a minor patient reaches age 12, full proxy access will be automatically revoked on a minor's MyChart account.

Proxy Access

A parent or legal guardian of a minor child (up until the child's 12th birthday), or the legal guardian or holder of a healthcare power of attorney for individuals age 18 or older, may request proxy access to another individual's MyChart account. This is accomplished by completing the MyChart Proxy Access Form and submitting it to SCL Health. Such access will only be granted to parties requesting proxy access who can demonstrate the legal right to the patient's medical information. A copy of each relevant supporting document, such as a birth certificate, court order or healthcare power of attorney, must be submitted with the MyChart Proxy Access Form. If the proxy's legal relationship with the account holder changes, the proxy must obtain and complete a MyChart Proxy Access Revocation form and submit it to a SCL Health Care Site Health Information Department or to an SCL Health physician office. SCL Health reserves the right to revoke proxy access at any time for any reason.

Levels of Proxy Access

  • Adult-Adult:Full access to another adult's or emancipated minor's MyChart record will only be granted where the proxy has documented legal authority to make healthcare decisions. Proof of legal guardianship or healthcare power of attorney is required. For other adults, the record may be accessed through the health information management system in accordance with the Patient Access to Protected Health Information Policy.
  • Parent-Child:Parents have full access to their minor child's MyChart record. This access will automatically transition to Parent-Teen access when the child reaches age 12.
  • Parent-Teen:Full or partial access to a minor child's MyChart record may be granted when the child is between ages 12 and 17 (inclusive). Both parent and teen will have full or restricted MyChart access. Access will be automatically terminated when the teen reaches age 18. Limitations in MyChart may not affect the proxy's legal right to access their child's record through Health Information Management.

E-Mail Privacy

Patients who are users of MyChart should be aware that they will be notified via email when there is new medical information to be viewed on MyChart. This means that any person with access to a patient's email will be able to see this notification. This could include the patient's spouse, employer, or anyone else who can access a patient's email account. Although no private medical information will be sent, the notification that new medical information is available by accessing MyChart may be information that a patient would not want others to know. Thus, patients should take this into account when providing an email address.

Please know that if you send us an email communication, it may be shared with the SCL Health staff that assists the physician in providing the patient's medical care. A patient's confidential medical information on MyChart will be accessible only to appropriate clinical staff.

Indemnification

You agree to defend, indemnify and hold harmless SCL Health and its subsidiaries and other affiliates, and any of our successors, assigns or licensees, together with any of their respective associates, contractors, officers, and directors from all liabilities, claims, damages, losses, judgments, cost and expenses, including attorney's fees, that arise from your use or misuse of MyChart, or any breach or violation of these Terms of Use or any other term or condition contained in MyChart. SCL Health reserves the right, at its own expense, to assume the exclusive defense and control of any matter otherwise subject to indemnification by you, in which event you will cooperate with SCL Health in asserting any available defenses.

Rights, Remedy and Termination

SCL Health reserves the right, in its sole discretion, to terminate your access to all or part of MyChart, with or without notice. These actions are in addition to and not in lieu or limitation of any other right or remedy SCL Health may have available at law. Further, SCL Health shall not be liable to you or any third party for any such termination or discontinuance.

Security and Confidentiality

We afford the same degree of confidentiality to medical information stored on MyChart as is given to medical information stored by SCL Health in any other medium. SCL Health is committed to protecting the confidentiality of your medical information. We limit SCL Health employees' access and ability to enter or view information based upon their role in your care. Firewalls, passwords, encryption, and audit trails are further used to safeguard your information. We shall identify the records released and note the time and date of access each time a patient or proxy accesses MyChart. We have taken steps to protect all information received from our online visitors against unauthorized access and use.

For other than general information viewing, MyChart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Google Chrome, Edge, Firefox, or Safari). Our SSL web server uses authentication to provide a high level of encryption technology security.

You can tell when you have a secure web connection by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data has been encrypted in order to protect it from access by unauthorized individuals. You can tell whether you are truly connected to SCL Health by viewing the digital certificate. This certificate verifies the connection between the SCL Health server's public key and the server's identification, ownership, and control.

Privacy

The terms of the SCL Health Privacy Policy and the Notice of Privacy Practices apply to your use of this Portal and are hereby incorporated. The Privacy Policy tells you how SCL Health collects, uses, protects, shares, and controls information about you related to your interaction with this Portal. Additionally, each affiliate of SCL Health that provides healthcare services maintains a Notice of Privacy Practices (NPP). The NPP provides information about how the applicable SCL Health affiliate might use and disclose your protected health information ("PHI"). Please view the NPP’s here.

Electronic Signature

You consent to receive communications from us electronically. You agree that (a) agreements and consents can be signed electronically, and (b) notices, disclosures, and other communications that we provide to you electronically satisfy any legal requirement that such notices and other communications be in writing.

Additional Terms of Use for Telemedicine Non-Urgent Care Visit (video visits)

Who can engage in a video visit? Persons of all ages are permitted to engage in a video visit with an SCL Health employed physician or advanced practice nurse.

Is there a restriction on video visits in certain states?

IF YOU ARE A RESIDENT OF COLORADO, EACH TIME YOU THE USE VIDEO VISIT SERVICE, YOU UNDERSTAND AND AGREE THAT YOU MUST BE PHYSICALLY LOCATED IN THE STATE OF COLORADO, AND YOU ACKNOWLEDGE THAT YOU ARE PRESENTLY LOCATED IN THE STATE OF COLORADO.

IF YOU ARE A RESIDENT OF MONTANA, EACH TIME YOU USE THE VIDEO VISIT SERVICE, YOU UNDERSTAND AND AGREE THAT YOU MUST BE PHYSICALLY LOCATED IN THE STATE OF MONTANA, AND YOU ACKNOWLEDGE THAT YOU ARE PRESENTLY LOCATED IN THE STATE OF MONTANA.

IF YOU ARE A RESIDENT OF WYOMING, EACH TIME YOU USE THE VIDEO VISIT SERVICE, YOU UNDERSTAND AND AGREE THAT YOU MUST BE PHYSICALLY LOCATED IN THE STATE OF WYOMING, AND YOU ACKNOWLEDGE THAT YOU ARE PRESENTLY LOCATED IN THE STATE OF WYOMING.

What is a telemedicine video visit? Telemedicine involves a secure, online exchange of medical information between a patient and a healthcare provider for purposes of diagnosing, consulting and/or treating certain non-urgent conditions, such as an allergic reaction, cold/flu, cough, pink eye, rash, fever, sore throat, or urinary tract infection. The healthcare provider is located in a place separate from the patient (who may be at home or the office) and the video visit is conducted via secure live two-way audio and video via the MyChart portal. At any time during the video visit, the provider may terminate the video visit and advise the patient to seek treatment from an in-person provider, or if symptoms described are emergent, to seek care from the closest emergency room.

Does a video visit create a doctor-patient relationship? By receiving the video visit, you are entering into a physician/patient relationship with the physicians (and/or advanced practice nurses or physician assistant) employed by SCL Health (collectively referred to as "SCL Health Physician"). Your use of the video visit is not meant to take the place of your relationship with your regular healthcare provider. An encounter summary will be provided to you at the end of each encounter. If you wish to share any information with your primary care physician or other healthcare provider, you have a right to access and request copies of your medical records, as allowed under applicable law. Neither SCL Health nor SCL Health Physicians will undertake to submit or provide any information to your primary care physician or any other healthcare provider. If you regularly see a SCL Health Physician, the information from your video visit will be included in your medical record and reflected in MyChart for review and consideration on your next visit.

Content of Communication You understand that you are responsible for your communications during your video visit. You will not:

  • communicate material that is obscene, defamatory, threatening, abusive or hateful
  • communicate sexually explicit images
  • impersonate another person

Follow-Up Care When engaging in a video visit, you are receiving services from an SCL Health employed physician, physician assistant, or advanced practice nurse. If you require follow-up care, your SCL Health practitioner may recommend other SCL Health employed practitioners or affiliated facilities, which are known to your practitioner and which offer the advantage of continuity of care. However, you are the ultimate decision maker when it comes to your care. You are free to select any healthcare provider of your choice and SCL Health does not undertake any obligation to determine whether and to what extent any follow-up care furnished at an SCL Health facility may be covered by your insurance.

Consent for Care via Telemedicine Services

  • I acknowledge that, while telemedicine can be used to provide improved access to medical care, as with any medical procedure, there are potential risks and no results can be guaranteed or assured. These risks include, but are not limited to: technical problems with the information transmission, equipment failures that could result in lost information or delays in treatment, and in very rare instances, breaches of privacy of personal medical information could occur despite security measures taken such as encrypting data, password protections, and implementation of other reliable authentication techniques.
  • I understand that I have a right to withhold or withdraw my consent to the use of telemedicine in the course of my care at any time, without affecting my right to future treatment. I also understand that I have a right to access all information resulting from telemedicine services.
  • I understand that I retain the option to refuse the delivery of healthcare services via telemedicine at any time without affecting my right to future care or treatment and without risking the loss or withdrawal of any program benefits to which I would otherwise be entitled.
  • I shall have access to all medical information resulting from the telemedicine services as provided by applicable law for member access to my medical records.

Acknowledgment of Financial Obligation (For SCL Health Patients Only)

  • For patients who have insurance, call the phone number on your insurance card to find out your cost share for video visits. If all or part of the cost of video visits is not covered, you will be responsible for the remaining balance, as permitted by our agreement with your insurance.
  • For patients who do not have insurance, the cost share for a video visit is $59.

If SCL Health is able to bill your insurer for the video visit, you hereby agree to assign to SCL Health all rights, title and interest in and to any payment due from any insurance policy, employee benefit plan or another responsible third party.

The Mobile App

We use background location services within our mobile app to allow patients to indicate that they’ve arrived for a scheduled appointment and automatically check in. If location services are not enabled by the patient, a manual check in process is available.

We use health and fitness API access to allow patients to sync Google Fit or Apple Health data into the app. This will automatically update provider-ordered flowsheets with data such as blood pressure, heart rate, and weight.
